Throughout the pandemic, Primary Care has continued to support their patients to the best of their capacity. As Wave 3 continues to put unprecedented pressures on the Acute Care system, a new challenge is emerging - providing structured and standardized approach to caring for patients with COVID@Home.
COVID@Home can be thought of as both an approach to caring for patients with COVID-19 in their homes and also a monitoring program with a slate of support services designed to care for the highest risk of these patients. COVID@Home is built upon the principle of supporting the patient and primary care provider relationship and the added value of continuity of care.
In this community webinar, our guest speakers will provide an overview of remote care monitoring as it applies to managing COVID positive patients as well as outline local COVID@Home programs available to you. A Q&A session will follow the presentations.
